Donating Your Vehicle is
Easy, Convenient and Fast!

REMINDER: Vehicles will only be picked up in Maricopa County.

1. Sign the Title as "Seller" exactly as your name is listed on the document. Your title may also ask you to print your name, please print your name as it appears on the front of the title. This must be done in the presence of a Notary. A notary is available in our office for your convenience M-F 8:30 a.m - 4:15 p.m.

NOTE: If you are not the registered owner on the front of the title, and you want to get the receipt in your name, the vehicle must be re-titled in your name as the current owner.

NOTE: If a lien holder is listed on the front of the title, the original lien release must accompany the title.

Click here for more instructions on how to transfer your title.

2. Complete the donor application. Click here for application.

3. Sign the Power of Attorney form. This form only applies to the vehicle you are donating and nothing else needs to be filled out except your signature. You must sign the form in the presence of a Notary.
